We’re excited that you’re taking the first step in connecting with a mentor who can support and guide you on your journey to medical school. This form will help us learn more about your interests and goals, and allow you to select a mentor who aligns with your needs.

Please complete the form in its entirety, including your selection of a mentor and the areas of support you’re seeking. Once submitted, the mentor you selected will review your responses and reach out to you directly within the next couple of weeks.

Please keep in mind that all of our mentors are volunteering their time to support aspiring medical students like you. We ask that you engage in this relationship with professionalism and respect. Any inappropriate, disrespectful, or unprofessional behavior will not be tolerated. Mentors maintain the right to discontinue a mentorship relationship if it becomes unproductive or otherwise problematic.
